Transcultural nursing is one of the most comprehensive and inclusive theories among all nursing theories. Another example is the proposition that one’s financial status may increase or decrease the likelihood of commitment because the ideal outcome may seem unattainable for people who are financially unstable.

As a Christian, I have cared for many Muslim patients. I like to talk about religion with my patient because I want them to understand that I recognize every religion. This assignment was performed using a teamwork-based approach. In the field of nursing and patient care, one of the greatest strengths in achieving set goals is teamwork. My personal value in working with others is to understand each other and work with everyone’s idea and effort where they are applicable. This assignment was one of the longest group papers in my course of this program. It heled me strengthen my patience in dealing with others (students, coworkers, and patients).

This project is added to the portfolio because teamwork has a significant impact in nursing practice. The saying that “no one is an island” is speak volume when it comes to nursing practice. My group worked together from the beginning of the semester to the end when the as the project lasted, we tolerate and understood each, and in the end finished the group paper with a good grade. So, this assignment is a testament of what teamwork can do in nursing.

An example is caring for a patient from a minority social background with the consideration that all humans are equal and deserves equal care and attention. All emphasis on nursing care is rooted in practicing safety and providing quality care. Providing safe and quality care is how nurses prevent harm and ensure that no error occurs to patients. In as interprofessional team effective communication is the key to preventing errors and promoting safety.
An example is timely updating the physician that a patient is hypotensive and may not need antihypertensive medication, or that the patient may need stat fluid bolus if appropriate. Another example is working with a physical therapist, updating the team that a patient was just diagnosed of bilateral thromboembolism, and may not need to be ambulated based on stat verbal order by the physical that is yet to be imputed. Working a harmonious way with other team of professionals is important in keeping patients safe.

My goal was to assess the need for proper transition to adulthood and to provide adequate education. To ensure that the need for collaboration among interdisciplinary teams, and adequate time to facilitate a proper and effective transition of patients with sickle cell disease from pediatric to adult health care is met.

This assignment is included, because it shines light to some areas and demography of patients with sickle cell disease who require thorough assessment, planning, and proper intervention before transitioning into adult health care and taking full responsibility of their care decisions.

I mate a young lady who just turn 18 ears old, and the who told me that her phone number was requested by the sickle cell team because she need to start assuming responsibility for her care decision making. She stated that “I refused” because I am not ready.

My proposition to this project was to allowing patients to transition to adult health care responsibility when both patients and parents are in agreement that patient is ready to take full responsibility of their care.

Regardless of which medical insurance the patient is using, a proper breakdown of healthcare cost must be presented. This is important because managing sickle cell appropriately and following recommendations reduces severity and frequency of crises and reduce cost.

Approach to the Assignment
My approach to this assignment was to analyze some of the ethical dilemma of face by nurses when providing end of life care. As a nurse, I have found myself in a situation where I was questioned by families why their loved one is taking too long to pass away. This does not mean that they want their loved one gone immediately, it is because of poor education, misinformation or misunderstanding of information provided by the palliative care team during decision-making consultation. My goal is that patients and families may receive adequate and proper education and awareness of the end of life decision. An education that explains process and expectations to avoid a situation of dilemma and misunderstanding between families and nurses.
Reason for Inclusion
This topic is included because ethical dilemma is one of the biggest challenges that nurses face when dealing with patients and families. In situations where a patient consults with several specialists or disciplines, every other question is directed to the nurse. Implementing orders can be challenging sometimes due to the nature of the situation, one’s personal feelings, and the family involvement. It is important that speak out when there is a shortcoming on the way palliative care measures are presented and implemented. Nurses are the closest to families and patients and understand their feeling, listens to their concerns and needs. So, it is sometime difficult to when is situation places a nurse into a dilemma.

The approach to this assignment was focusing on the impact of technology in healthcare and how patients access their healthcare record. As patients uses technology to follow up, make complaints about care receives, or give commendations on care they received in a hospital. Because hospital experience is very important in the outcome of care provided to patient, and the image of the hospital in question, understanding how online reviews affects hospitals image and recognition is very important.

It was an eye opener to understand the power of internet, and smart devices as it relates to healthcare information. This assignment and my findings allowed me to pay more attention to issue of patients’ health that may be considered “not serious” but has proven deadly. Examples are ensuring that patients void appropriate amounts of urine post operative and to ambulates post operative patients with the recommended hours as ordered.

It is safe to say that data acquisition is not only done by the health care team. Patients and family have access to some of their health records. If there is an inconsistency in the patient’s data page, they will see it. My goal is to provide appropriate evidence-based care with the understanding that most my documented care can be accessible by the organization and the patients. So, I ensure that ensure that the care I provide, and my documentation are consistent regardless of the social economic or educational background of the patient. Patient may not be educated enough to access their information online, but their families and relatives may.

A key component of nursing care is safety. Practicing and providing a safe care is paramount in the benchmarking system. Because most of the issues in patient care is related to safety, I have made is an ought to do routine to practice safety in providing care to my patients. By ensuring the at I double check patients information before administering care; ensuring the right interventions are provided; answering the call light timely; listening to patients preferences and concerns; end utilizing evidence based practice appropriately.

This assignment exposed me to some aspects of informatic in nursing that I never knew existed. My informatics knowledge has help me in utilizing available system information to deliver appropriate, faster, and effective care, and to reduce errors in my daily nursing care. Purpose of the Assignment
The purpose of the project is to examine a community health problem for a specific aggregate. The project is to be able to apply the nursing process to a vulnerable population within your community. In addition, the purpose of this assignment is to utilize the Health Planning Model to improve aggregate health and to apply the nursing process to the larger aggregate within a systems framework.
Approach to the Assignment
As a nurse and an educator who assess patients and provide education based on their health needs, this assignment was performed by assessing and identifying health need of my patients and families, and offers educational awareness. By understanding what patients and families consider their health problem, I was able to establish educational goal that is based on the palliative care.
Reason for Inclusion
This assignment is included because the assessment of palliative care knowledge exposed the psychological effects of poor communication, lack of information, and miscommunication among nurses, patients, and families. During this assignment, I was able to establish the reason why nurses, patients and families find it difficult to talk about bout palliative care. This topic is still being delicately discussed among patients, families, and the health care team. The understanding is still limited, as I continue to education the population within my reach.

Being a global citizen starts from home. As a registered nurse my contribution starts at bedside as I have come to understand that equity is not always practiced in health care. In relation to my discussion on palliative care, I found that the more education and better socioeconomic class more likely to a patient is encouraged to use palliative care alongside curative care. So, I have made it a mission to education my patients who are going through debilitating health problems that palliative care can be used alongside curative treatment. Education them that palliative care is based on patient’s need not diagnosis or prognosis.

I ensured that my patient understand that palliative care is not a death sentence and does not imply that the person has given up as people and public opinion may suggest. Because opinion, stereotypes, misconception, and misinformation are some of the factors that influence the use of palliative care. Education my patient population about the component and benefits of palliative care is important as more adult a living longer and going through health issue.

To ensure that I understood the extent of lack of knowledge awareness of palliative care, I asked around my patients, families, and colleagues about their understanding and opinion. Around the time of this assignment, I have two patients passed away with little to no understanding of the document they and their family signed. That made this topic a major clinical problem for me. I decided to ask my patient about their understanding of patientive care and explain in details to them what it is all about.

After reading through many article on the topic of palliative care, I was able to provide to patients and family a tailored plan that is based on their goal should they decided to consult for palliative care measures.

The impact of health disparities and social determinants of health can be detrimental to general health. When health disparities and social determinants are used in the analysis of someone with severe health conditions, the outcome is debilitating. In relation to an educated patient, from an upper social class who suffers from cancer or COPD, care plan, treatment regimen and attention to detail is much better compared to a patient from a minority class. As a nurse educator who also advocate for my patients, I ensured that I provided my patient with available resources and care options available to them.
